Description
This lecture will provide a comprehensive, clinically oriented overview of cemental tears, a relatively rare but often underdiagnosed root surface fracture that may masquerade as periodontal or endodontic disease. We will start by reviewing the epidemiology, predisposing factors and common tooth sites for cemental tears. We will then move into how cemental tears present — both clinically and radiographically — and how they may be misdiagnosed as vertical root fractures, periapical periodontitis, or periodontitis. We will explore diagnostic strategies, including the role of CBCT and exploratory surgery, with examples from case series. The treatment section will cover nonsurgical and surgical options and factors that influence prognosis and predictors of success or failure.
Learning Objectives :
- Recognize the clinical and radiographic features of cemental tears, and distinguish them from vertical root fracture.
- Explain the contributing risk factors, pathophysiology, and common locations of cemental tears, based on current evidence.
- Describe the evidence-based treatment options (surgical, nonsurgical) for cemental tears, and discuss prognostic factors affecting tooth survival.
